Strategic Plan

The Board of Directors for Healthy Carolinians of Carteret County determined that the following were the top five (5) health priorities that needed to be addressed in our community over the next five years:

1. Access to care
2. Obesity
3. Physical Inactivity
4. Drug/Alcohol Abuse
5. Mental Health

Following is a five year strategic plan developed by the Board to guide the community in addressing these issues:

Goal #1

Establish HCCC as the community facilitator for health promotion and prevention.

Objective 1

By June 30,2011, maintain a full board of directors and coordinator position.

• Develop a Policies and Procedure Manual
• Evaluate Need for Executive Director

Objective 2

By June 30, 2011, communicate HCCC goals and objectives to relevant and prospective partners.

• Establish a five-year strategic plan
• Develop a communication plan for community stakeholders
• Conduct an Annual HCCC Conference that highlights annual progress and success

Objective 3

By June 30, 2011, maintain the Healthy Carolinians (HC) certification.

• Submit 2006 Recertification Application to Office of Healthy Carolinians

Objective 4

By June 20, 2011, sustain administration and program funding for the organization.

• By November 30, 2006, identify program focus areas and identify new funding sources that align with HCCC goals and objectives.

Goal #2

Support programs and/or interventions for eating smart and moving more.

Objective 1

By June 30, 2011, there will be no increase in adults, youth and children who are classified as overweight or obese.

Objective 2

By June 30, 2011, at least 46% of adults will get recommended amounts of physical activity each week and fewer than 15% will report no leisure time physical activity.

Goal #3

Develop systems that promote access to health care, programs and services.

Objective 1

By June 30, 2008, establish NC CareLink, a nine county web-based Information and Referral System through DHHS, NC CareLink.

• Establish a website — www.healthycarteret.org
• Establish NC CareLink Regional Advisory Council
• Re-establish a telephone crisis line – Helpline
• Establish and provide administrative support for patnerships that create Volunteer Carteret and the Carteret Resource Connection

Objective 2

By June 30, 2009, establish a plan that addresses transportation issues for vulnerable populations in small, rural communities that provide them access to health care.

• Partner with community organizations to research, review and evaluate other model programs that address transportation issues in rural communities

Goal #4

Establish a healthy youth, healthy community initiative.

Objective 1

By June 30, 2007, establish an Alcohol and Substance Abuse Coalition that assesses mental health and substance abuse services in the county.

Objective 2

By June 30, 2008, launch a county-wide strategic plan for healthy youth, healthy community initiative.
